If you wish to have the report faxed to you, you must provide the FAX number on the form. Otherwise, we will phone you when the report is ready for pick up.

Voicemail is available 24-hours a day.
You will need to provide the date, approximate time, and address (or intersection)
of the incident.
We
WILL NOT
fax or mail medical reports requested by patient or parent/guardian of a minor patient. These reports must be picked up in person. Patient picking up a medical report must present a valid photo ID. Parent/guardian of a minor patient must present a valid photo ID and a birth certificate of the minor patient.
We
WILL
fax or mail medical reports to parties that submitted an original notarized
medical release form HIPPA
by mail.
Reports can be picked up in our office during regular business hours.

Who can request an incident report
